This program focuses on re-engaging individuals with HIV/AIDS who face accessibility, language, and financial barriers to engage in adequate, essential medical care. Services are funded by The Pennsylvania Department of Health, the Bureau of Communicable Diseases, the Division of HIV Disease, and the Jewish Healthcare Foundation. Services include:

  • Intensive outreach efforts to high-risk individuals, clients with comorbid conditions, and clients newly diagnosed with HIV
  • Client care services
  • Interpretation/translation services
  • Door-to-door medical transportation
  • Wraparound service plan and referrals (behavioral health and drug & alcohol services)
  • Medication adherence
  • Assist with obtaining health insurance coverage (Special Pharmaceutical Benefits Program, Medical Assistance, ACA, Medicaid, Medicare, etc.)
